Al-Malath Hospice provides care by a team of skilled professionals and volunteers who have as their priority the comfort and care of the patient and family.
Besides the Patient, who makes personal choices regarding care and requests the services s/he needs directly or through a designated family member, the Hospice team includes:
For Patients And Families
Patients may refer themselves personally, or be referred by their family doctors, a family member, or a friend.
When referral is made, the Malath Patient Care Service Coordinator contacts the patient’s doctor to secure a complete history and treatment record, in order to determine whether or not the patient’s needs can be met by Al-Malath team. Also to obtain permission for an initial assessment visit After the initial coordination, orders are given to the nurse in charge, who in turn prepares the initial nursing arrangements after the assessment visit.
The care plan is prepared by Al-Malath hospice team and updated periodically according to the patient’s emerging needs and problems. A separate care plan is also designed to the family members who are in need for counseling, support or training.
This procedure initiates a plan of care and ongoing coordinated home visits from the health care team.
